Designated 志望理由書 form; answer the Mathematics department questionnaire on the form (include the questions).
Graduation / degree certificate
Bachelor's graduation or expected-graduation certificate; overseas applicants also submit degree or expected-degree certificate (may be combined). Certified translation if not in Japanese or English.
Undergraduate transcript(s)
Official bachelor's-level academic transcript(s) from the degree-granting university (originals; certified English or Japanese translation if not in Japanese/English). Include prior institution transcripts if transferred. Graduation or expected-graduation certificate and, for overseas universities, degree or expected-degree certificate are also required.
